Admission and Health Related Documents:

The School Health Department of the Ministry of Public Health, Kuwait has the following health policy for admitting students to school in Kuwait.

New Students require a full medical examination at a Government Health Center designated by the Ministry of Health Kuwait. A letter of introduction will be supplied by the school admissions office administrator. Having completed this examination, the doctor will issue a school health file which the parent should submit to the admissions office.

Students who are transferring from another school within Kuwait should obtain their medical records from their previous school and submit them to the School Administration Department (Admissions Office).

The following forms should be completed by the parent when applying for enrollment with the admissions office.

Medical Information Form:

This form applies to the medical history of the student and will be updated accordingly throughout the school year.

Informed consent of the parent or legal guardian:

This document gives authority to the medical provider selected by the Universal American School, to administer or secure proper treatment for a student who requires medical intervention while under the school’s jurisdiction, and beyond the routine health services available at U.A.S. if outside referral is necessary.

Emergency Contact Information Form:

This form should be filled in by the parent/guardian annually and renewed in the event of a change of phone number, address or emergency contact person.

Vaccination Information for students entering school in Kuwait:

Having completed the standard medical examination by a Ministry of Health Doctor, a Medical Certificate will be issued as verification that required vaccinations have been completed.

It should be noted that the Kuwait immunization requirements may vary from the vaccination schedule required by another country. The Kuwait Schedule is recommended by the World Health Organization (W.H.O) and The Ministry of Health Kuwait.

All Vaccinations for students 5years old or younger are given at the Kuwait Government Health Clinics. There are no required vaccinations for ages 6 – 9 years. From age 10years to 18years, the required vaccinations are given at school.

Vaccinations are not administered to any student at the Universal American School without the prior Consent of the Parent.

School Health Policies and Advice:

Medical Records:

To assist the Clinic staff in the care of children at school, we ask parents/guardians to complete the student medical information form upon admission and notify the clinic staff of any significant changes in health, which may occur during the school year.

Parents of students already registered with a pre-determined Medical Condition are kindly requested to submit an updated medical report at the beginning of each school year.

General Illness:

  • Please, do not send your child to school if he/she is ill and has a temperature of 37.7C (100F) or over.

  • If your child is absent due to illness for more than one day, a doctor’s report will be kindly appreciated, as this will help in the care of your child on their return to school.

Communicable Diseases:

Exposure to a variety of communicable diseases in a school population is inevitable. In order to safeguard the school community from the spread of certain communicable diseases, we ask the parent to inform the Health Clinic Staff and/or Principal as soon as practical if a child is infected with a communicable disease.

In such cases, a medical certificate is required when the student returns to school.

Medication:

The Universal American School has specific requirements for the administration of medication at school. For safety reasons, the parent or an authorized adult must deliver the medication to school personnel for Nursery through, Elementary students, rather than sending the medication along with a child or left in school bags.

  • All medications must be brought to the School Clinic before the start of the school day.

  • All medications must be clearly labeled with the student’s name and grade, the dosage prescribed and the time to be dispensed.

  • For students on continued daily medication, a doctor’s letter must be submitted, stating the student’s medical condition, and detailing the time and dosage of the listed medication.

We request your co-operation with the above, so that we can provide a safe and healthy environment for your child.

Diet:

We request your assistance in helping us to educate the children to eat a healthy diet.

Hence; we strongly recommend that parents do not send, sweets, candy, gum, chocolate, soft drinks or crisps to school. Not only, do they lead to poor diet habits but also pave the way for dental decay, obesity, low levels of concentration and/or hyperactivity.

Peanut and Nut Alert:

The Universal American School works towards helping our school become free of peanuts and foods containing nuts, by discouraging our children from bringing these foods to school.

Besides a choking hazard in children, food containing nuts is a precursor to anaphylactic shock in people who are hypersensitive to nuts.

Recommended Sleeping Hours:

Age 3 – 5 years:    Recommended 10 – 12 hours a day.

Age 5 – 10 years:  Recommended 10 hours a day.

Older children:     Require 8 hours plus.

A regular bedtime renders a good sleep pattern.
