HEALTH INFORMATION AND MEDICAL FORM
Parents:
The Universal American School Health Clinic requires a Health History on each student and recommends that parents notify the School Health Clinic of any significant changes in health, which may occur during the school year. (Kindly complete the attached form)
Parents of students already registered with a pre determined Medical Condition are kindly requested to submit an updated medical report at the beginning of each school year.
General Illness:
- If your child is ill, has a temperature of 37.7C (100F) or over, please, Do Not send your child to school until your own doctor has approved.
- If your child were absent due to illness for more than one day, a letter from you or a doctor's report would be kindly appreciated, as this will help in the care of your child on their return to school and in the prevention of CROSS infection.
Medication
If medication is required during school hours the following guidelines are recommended.
- All medications must be brought to the School Clinic before the start of school.
- All medications must be clearly labeled with the students name and grade, the dosage prescribed and the time to be dispensed.
- For students on continued daily medication, a doctor's letter stating their medical condition must be submitted, detailing the time and dosage of the listed medication.
